	Student file information is automatically rolled over from one school year into the next by our computer system, and changes need to be specifically entered into the programs in order to take effect.
	Student must be 15 years old to begin the classroom portion. Student must be 15 yrs and 6 months old & must be attending the classroom portion plus pass the written exam to receive a Temporary Driver License.
	It is the Parents’ / Guardians’ responsibility to keep this information current.
	Needs to be on hand in the office in case of emergency .
	Card for student health examination.
	The purpose of this form is to measure compliance with the law and will be used for that reason only. If you have questions on immunizations or how to complete this form, contact your child’s school or local health department.
	It is the policy of Catholic Central High School to NOT release any individual student’s, parent’s staff’s, or faculty member’s personal information without the individual(s) and in the case of minors, from their parent or guardian, written permission on file in the school office.
	Parent/Guardian Prescription Consent form needs to be on hand in the office to prescribe medicine to a student.
	Free and Reduced Price School Meals
	Community service form to be completed and handed in to the student's theology teacher.
	Students must sign this in order to have access to all the technology on campus.
	Necessity for a student to go to Catholic Central.
